Safe Discharge Home
Superior Senior Resources can arrange for reliable transport from hospital back to the home. Our hospital discharge services make the transition simple and safe. The package provides:
- Food Shopping (prior to discharge --providing at least 3 days’ worth of basic, everyday food items)
- Pick Up and Delivery of Medication
- Assured Delivery and Set Up of Durable Medical Equipment
- Final Safety Check of Home (removal of expired food items, flagging broken appliances, etc.)
- Scheduling of Doctor's Appointments within first 10 days of discharge (and the option to accompany clients to appointments accordingly)

Home Wellness Assessments and Safety Checks
Initially, we evaluate the following categories (and assess the results) to make our determination for which living option will be best for your or your loved one. Our Home Wellness Assessment evaluates the client's:
- Necessity of Durable Medical Equipment
- Food (current status and the client's ability to resupply)
- Medication
- Cognitive Capacity
- Doctor/Wellness Visits
- Vehicle Safety
- Family Communication
- Home Repair Issues
- Ability to perform Activities of Daily Living
- Dressing
- Bathing
- Toileting/Continence
- Meal Prep
- Grooming
- Transferring (as needed)
- Clutter/Hoarding concerns
We also offer routine Home Wellness Safety Checks on a daily, weekly, and monthly basis to ensure the results of our initial assessment have not changed.
